Acupuncture in the Management of Temporomandibular Disorders : Evidence, Mechanisms, and Clinical Applications

Temporomandibular disorders (TMD) encompass a cluster of musculoskeletal and neuromuscular conditions affecting the temporomandibular joint (TMJ), masticatory muscles, and adjacent craniofacial structures. Characterised by chronic facial pain, masticatory muscle tenderness, joint sounds (such as clicking or crepitus), and restricted mandibular range of motion, TMD represents a leading cause of non-dental chronic orofacial pain.

While conventional frontline therapies include non-steroidal anti-inflammatory drugs (NSAIDs), occlusal splints, and physical therapy, an increasing body of clinical research has investigated acupuncture as a non-pharmacological, minimally invasive modality for managing TMD-related symptoms.

Clinical Efficacy and Evidence from Trials

Systematic reviews and clinical trials indicate that acupuncture can provide significant short-to-medium-term symptom relief, particularly in patients presenting with myofascial pain and muscular subtypes of TMD.

  • Pain Reduction: Meta-analyses of randomised controlled trials (RCTs) have shown that manual and laser acupuncture achieve statistically significant reductions in subjective pain intensity as measured on Visual Analogue Scales (VAS) compared to baseline and sham controls (Di Francesco et al., 2024). Recent sham-controlled trials demonstrate that 4 weeks of structured acupuncture treatment leads to notable improvements in pain intensity that persist through an 8-week follow-up period (Liu et al., 2024).

  • Restoration of Mandibular Function: Beyond analgesia, acupuncture interventions are associated with objective functional improvements, such as increases in maximum unassisted mouth opening and decreases in muscle tenderness upon palpation (Di Francesco et al., 2024; Liu et al., 2024).

  • Quality of Life and Psychological Metrics: TMD is frequently comorbid with emotional distress and sleep disturbances. Clinical evaluations show concurrent improvements in psychological metrics, including stress, anxiety, and sleep quality scores among patients receiving verum acupuncture compared to sham procedures (Liu et al., 2024).
